OPEN ENROLLMENT CHANGES FOR THE 2010-2011 BENEFIT YEAR

The 2010-2011 Section 125 Cafeteria Plan year will begin on 08/01/2010 and will go until 07/31/2011. All changes that were made during Open Enrollment will go in effect 08/01/2010, with the exception of Blue Cross/Blue Shield and Texas Life Insurance. Because these two insurances are pre-paid, the changes for these products will go in effect on 09/01/2010. All new deductions will be reflected in your August pay check(s). Please see the chart below that shows when your changes to your insurance(s) will take effect and the pay periods in which we will begin deducting for.

You may review your benefits online, by logging on to www.comalisdbenefits.com. Once there, you will click on “View Benefits”. Your username is the first 6 letters of your last name, first initial of your first name and the last 4 numbers of your social security number in all lower case with no spaces. Your password was defaulted to your six digit date of birth, for example 020560, unless you specified otherwise. You will have the option to change your password once you have logged into the system. Under the “Employee Menu”, click on “Benefit Plan Information” and then “Consolidated Enrollment Form”. From here, you will be able to view your benefits. YOU MAY ONLY VIEW YOUR BENEFITS; YOU WILL NOT BE ALLOWED TO MAKE ANY CHANGES ON-LINE.

Please note that if you will be new to the UNUM disability, the Allstate Cancer, and the Ameritas Dental Buy Up plans effective 08/01/10, pre-existing conditions and waiting periods will apply. UNUM Disability -the pre-existing condition exclusion will not cover any disability that begins in the first 12 months after your effective date of coverage that is caused by, contributed to by, or resulting from a pre-existing condition. Pre-existing condition means a condition for which you received medical treatment, consultation, care or services including diagnostic measures, or took prescribed drugs or medicines in the three (3) months just prior to your effective date of coverage. Allstate Cancer –The policy and riders contain a 30 day waiting period that begins on the effective date. No benefits are payable for any covered person who has cancer or a specified disease diagnosed before coverage has been in force 30 days from the effective date. Ameritas Dental Buy-Up Plan- one year waiting period for TYPE 3 (major services and orthodontia).

Please check your August 2010 paycheck(s) to make sure that we are deducting you for the correct benefits.
